Are you using Rifle Primers?

They are stronger than other primers.

This is my question as well. 
445 requires a large rifle primer according to the Hodgdon online data

If you are shooting a lot of shorts, ie 38s in a model 40 or 44 mags in a 445, you could be getting a carbon ring in the chamber from the shorts that us preventing the full size SM cartridges from fully seating into the chamber. If they are a bit proud, the case head will protrude and drag on the recoil shield and cause issues similar to what is described.

 

A good cleaning with a brass brush in a drill and some solvent (non flamable kind) will remove that ring.  

Anything worth doing is worth doing well.

Exactly what I did before the shoot because I was shooting 44S and 44Ms.   This was 2nd shoot since acquiring it.

I think I need to take it apart and polish/sand the firing pin as it has a slight mushroom on the tip.   Do I sand back to a fine point?   or make sure the tip is flat?

I think you're correct in thinking it is a smooth radius, a point would pierce the primer,YIKES!!!!!!!wow

Could be primers, there is some difference in hardness between brands. Can't offer suggestions on brand as I've been out of the game for ages.

CCI is always my go-to brand for primers, if this ends up being a primer issue. I've never had a problem with them.
