I've been experiencing an intermittent failure to fire when shooting double action, it does fire the on a second strike. Single action is fine. Also, it's done it with two types of ammo.

It may be the strike. In single action the hammer pin strikes the primer dead center. In double action, it strikes off center.

I'll try new springs, but any other suggestions would be very welcome.

Happened to me today. 100% in SA, 50% in DA. Backed out the grip screw 1/2 turn, and it was "Rock 'n Roll" from there on. Try this first, it may be an easy fix. Although your issue sounds like a timing problem, consider this:

If your hammer is not reaching full cock in DA, it's going to drop slightly early, falling while the cylinder is still in rotation into full lockup. While this sounds pretty scary, the difference between slightly off center and on center on the primer is very small. Failure to Fire in this case is probably due to less than full striking force of the firing pin, not off center. If your DA strike is wildly off center it may be a timing issue, but the grip screw issue is easily diagnosed in a few minutes, and the much more common cause in a DW.

Thanks Steve,

I'm using a shorter grip screw than standard, 1.25" instead of 1.5". And it's done it with both the grippers and another aftermarket grip.  I could shoot the grippers without any screw at all to check because they are very tight, but I've a feeling that it's something else.

Good luck on this, I just want to make sure you try the easy solution first.

Sorry to bring up an old post but did you ever figure this out? I am in the same situation. Single action fire is 100% but DA is way off like 25-50% of the time. I installed all new springs and played with the grips screw. The DA strike is VERY faint on the primer, but dead center. SA is nice and deep.

NO, I never did. It fails about one round per cylinder. The strikes are hard but slightly off center. They are centered in single action. So we seem to have different problems. I should do some investigation to see if it's just one chamber that is the problem. But to be honest I don't use double action much - usually just to see if it's still a problem.

I've backed out the grip screw and even used a shorter one. I've been meaning to change the springs but haven't got round to it. I'm sorry I can't be more help.
