When I looked at the off center strikes, the firing pin was striking high relative to the center of the primer. It was locking up just fine before the hammer fell. My non-gunsmith brain thought timing issues (cylinder rotation) would manifest itself as strikes right or left of center. The high strikes I thought as a loose manufacturing tolerance. Didn't see anything that looked bent, so I'm guessing that dimensionally the cylinder was just built a bit low relative to the firing pin. Fired another 100 rounds double action through her today without failure.

Another thing to check is cylinder end play. If the cylinder has too much movement front to back then the movement will absorb some of the hammer momentum and cause light strikes. Check this by pulling hammer back to lockup, then hold hammer & pull trigger, keeping trigger pulled back, let hammer go forward slowly while holding with thumb. Keep trigger held back and try to move the cylinder. Slight rotational movement is Ok. The less front to back movement the better. If it feels like more than a few thousandths front to back then you need to shim the cylinder. Triggershims.com has shims. It is easy to put them in. Good luck.
