Is this a timing issue?  Something I can fix or is it gunsmith time?

The cylinder misalignment can be seen on the left side as thin black crescent.

It's the same at all indexes.  Brand new factory 4" barrel - I have not checked this with the original 6".

With no photos to go on we can only speculate. Assuming you’re talking about looking down the barrel with a bright light; yes, it would seem that the cylinder rotation is coming up a little short. When you say “indexes” are you certain the cylinder is locked in place by the bolt, or do you have to turn the cylinder by hand to complete the lockup, and if this rotation is then complete, is proper alignment achieved? If this is the case, replacing the hand should remedy the problem. However, a very badly bent crane could also cause the mis-alignment you’re speaking of with the bolt locked in bolt cuts. If a few very clear photos could be submitted it would greatly assist with a correct diagnosis.

My mistake - I wasn't thinking clearly (for reasons) and to be "safe" I decocked the pistol before pointing it at myself.  
Of course that meant the cylinder wasn't locked up.

Mea culpa!
