Anyone have a fail to fire issue with your model 15?
I have a Monson 15-2 which I have owed for 25+ years. I recently purchased a 2.5" barrel & combat grips. Took it the range with a box of Fiocche 38 specials and found it to be quite accurate and easy to control firing 2-3 shot double action fire. Went back to the range and tried a box of 158 gr full house magnum rounds. BTW full house mags from a .357 snubby are good fun and really wake up the other shooters at an indoor range. However I had like 9-10 fail to fires. I assume the hammer spring is original although I did purchase used. I have never had a fail to fire up to this point however most of my shooting prior has been single action with an 8" barrel. Nothing seems out of sorts with the action. Did I get a bad box of ammo or should I be checking anything. Ammo was Independance brand, never used it before. Since I have a 44 for hunting and long range use this gun is being converted to home defense and combat style shooting use. So the fail to fires are not acceptable.

Definitely try some different ammo. There is also some discussion on DWF (somewhere) about firing pin maintainence-try a search

Absolutelly check some other ammo. It is possible that Independance uses some very hard primers. I have had another non DW gun that had the same issue with frequent yet random misfires and it ended up needing the hammer spring replaced. But cheapest is check the ammo first.

The guys have the three I would check: ammo, firing pin, grip screw.
I would be most suspicious of the grip screw though if you just changed to the combat grip and noticed the problem. The countersunk hole could be too deep. Put your target grip back on and give it a try. A short grip screw from DW Norwich might be the solution.
There was also just a post about the grip screw seat being in too far - my model 15 has ~2.5 threads showing.
1/16" punch takes the firing pin out. Check sping and check hole / pin for gunk, free movement.

Thanks for the input, I do have the short grip screw with the new Houge grip. The grip screw seat has always been about halfway out. I will try other ammo when I can actually find some to buy, it's getting stupid around here already. The first box of Fiocche 38s worked fine so i'm really thinking it was an ammo issue however I will remove and clean the firing pin.

Good luck, hopefully it's just your ammo. But checking the other stuff will be a good thing to do also.
Last weekend, I had a FTF on one round out of 70, with my Model 15. It happened when I was firing in double action. I waited a sec, then opened the cylinder & found a light primer strike...spun the bullet 180º in the hole & it fired in single action.
My firing pin also doesn't strike the primer in the center, I don't know if that has anything to do with it.
