Ill clean the cloth ( cloth is old so ill try and be gentle ) and refinish the grip. Any good stain matches or would a Walnut or Old American work? What is an LB grip?
Pop the medallions out from the inside - an allen wrench works well to pop them out. Strip off the old finish with a finish stripper. You can soak the grip for 15 minutes in a small can then scrape off the old finish. You can rinse it off with water but don't soak it. Once you have the old finished removed, let it dry out then do a light sanding over the entire grip to smooth it out. You'll need to scrape around the inside of the medallion inlet and clean it out because it swells a little and the medallions will be very tight to press back in. Also ream out the hole for the medallion pin a little bit. Use MinWax Wipe-On Poly, Clear Satin. You'll need to apply about 5 coats (minimum 3 coats). If you do a good job on it, often the grip looks nicer than the original finish because it allows the wood grain to be more visible.
EDIT: - Just saw that you listed it on GI. It's aggressive pricing considering the "Freckles" are rust spots and will require a professional refinish which isn't cheap. Forget all the refinishing info. I posted - it looks like you're good to go selling "as is".
