Yes, to both questions. The foam in the cases was cut to the shape of the gun, left side up, barrel pointing left, with additional cutouts for 3 barrel/shroud combinations in the case of the 357; and, generally one or two additional cutouts for barrel/shroud combinations in the case of the 44. Additional cutouts were for spare grip and a rectangular space for miscellaneous such as buckle, patch, tool etc.. The earliest pacs did not have cloth covering the foam. Later they had yellow cloth covers of top and bottom foam - most commonly seen for sale. As time went on, the type of case, and cloth changed.

I don't think there has been a serious compendium of case types done on the forum as yet. It would be an interesting project.

That certainly looks like an original DW case, the foam looks to have all the cutouts in the right places. I think it may have some new cloth, however.
I went to a bookstore and asked the saleswoman "Were is the Self Help Section?" She said if she told me, it would defeat the purpose.

Definitely a DW hard case. Cloth probably replaced. From the pics I would guess it is for a model 44 or 744. Primarily because the 357 (15-2) came with more cutouts for barrels down the left side.
It would be revealing to see a photo of the bottom foam without the cloth cover. That would help to determine the original use. Of course it was possible to special order many different combinations so who knows. You can always make a new foam insert for the bottom to fit your own preferred combo.
