Obviously an over pressure failure, now what caused that, ie: barrel blockage, double charge,

bullet set back, ect.... your guess is as good as theirs at this point.

Very rarely will you ever hear the real story.  It's almost always the firearm, not the owner....  the fact that the damage is in or around the forcing cone makes me think timing or prior squibs. 

Technically, the glass is always full; half liquid, half air....

A squib is a round that has little to no powder charge.  The primer will push the bullet into the barrel but it will then get stuck there.  If another round is fired with the barrel blocked by the previous round, extreme pressure results that has to go somewhere.  That usually causes something to fail, barrel, cylinder, frame etc. 

Thats why its very important to clear a squib BEFORE firing another round.

Its usually fairly obvious when you have a squib, there is little to no recoil and little to no sound.  BANG...BANG...popwink.  

I had one once in a 445 supermag, it was OBVIOUS what had happened.  It felt like I shot a cap gun.  I usually  have a cleaning rod in my range bag so I just tapped the squib out of the barrel and went on. 

After that, I got a low powder alarm for my press to catch any future squibs before seating the bullet.

I agree it is stupid to knowingly fire another round immediately following a misfire or squib load

but shooters that are firing at a fast pace repetitive rate get in the zone so much that

by the time the failed round registers in their head they have already broke the trigger

on another round and the "Oh shit" races through their brain.

 

On a semi auto sometimes the failed load is low enough that it does not rack the slide

and eject the spent case but sometimes it does. With a revolver shooter, shooting full

speed in double action, there is nothing at all to stop the next round being shot.

 

I have been next to shooters when a one shot sounded like a CB cap and they fired

another so fast before they stopped to check the gun. It can happen and when that

shooter and you look at each other the "Oh shit" look is unmistakable.
